Clinical Study of Regional Arterial Infusion Therapy on the Cancer of the Head and Neck


Abstract
The treatment of head and neck cancer is important in otolaryngological field but very often it is very difficult to manage and the prognosis is not good. So removal of the tumor mass is accompanied by the removal of the important part of the head and neck, that irradiation therapy takes a good role instead of surgery. Recently in combination with surgery and radiation, regional arterial infusion of anticancer drug plays a good role on cancer of the head and neck. After this method was .first advised by Klopp in 1950, Sullivan(1959) and Corgill (1962) ¢¥contributed themselves to develop and apply in clinical use.
From Jan. 1972 to June 1976, 113 patients of head and. neck cancer were admitted to SNUH and 26 patients among them were treated¢¥ by 5-FU arterial infusion and other method. We obtained some good results and report it.
1) During 4 years and half, total in-patients were 2, 158 and malignant disease occupied 5. 2%.
2) Male to female ratio was 3.5 : 1 and average age was 53.8 years, male after age of 40 years takes a great part.
3) Among 113 patients, 46 cases (40.7%) were laryngeal cancer, 35 cases (31%) were nasal and paranasal sinus malignancies. Anthers were tongue cancer (7 cases), cancer of the oral cavity (5 cases, tonsillar, cancer (4 cases), palatal cancer (4 cases), mandible cancer (3 cases), ear cancer (2 cases). and others (8 cases.
4) 20 out of 26 patients who were treated by arterial infusion were nasal and Paranasal sinus malignancies. 2 tongue cancer, l .palatal cancer, 1 buccal cancer and 2 tonsillar cancer were also treated with infusion.
5) Pathologically, 22 out of 26 were squamous cell carcinoma.
6) In 11 patients, the tumor mass disappeared grossly, and in 5 patients marked regression of tumor mass was noted. But in 8 cases, there was no effect and 3 among . them aggravated.. We could not assess exactly in 2 cases because of poor charting.
7) Among 16 patients who showed good effect, 15 patients were treated early within 6 months without any previous irradiation.
8) 6 out of 8 patients who were not good were treated more than 6 months after onset and 5 patients among them received heavy previous irradiation. 9) Side effects of infusion therapy were stomatitis, cheek inflammation, alopecia, regional pain, headache, facial palsy, displacement of the tube during infusion, etc.
Troblesome bone marrow depression was not noted at all.
